When you see a blank question, it means that the user who asked it was deleted before you even saw the origional question. It was probably a spammer. I get a lot of those in my alerts.

To answer your questions:
Blank friend requests are from deleted users. There should be an option to delete it, which would dismiss it. It won’t come up again.
The blank question alerts: Are also from deleted users who you were friends with or questions that were deleted for bad content in the question or the comments. I will try to make it so those disappear but it could take a few weeks.

Also, if someone who was deleted wrote questions we don't necessarily want to delete them because the answers are good and you all worked hard to answer the questions.
