The black mark in the mouth may just be pigment coloring. Many of my cats have large black splotches in thier mouths and it’s normal.
If your vet has ruled out fleas and has used extensive diagnostics and year long diet testing with single protein source feeding and novel protein source feeding to rule out food alergies the next things I would consider the bald spot could be is over grooming or possibly ringworm.
