It certainly can be a serious problem!
Actually, when you asked this question back in October, Kat gave a great response. See it here: http://www.cats.com/question/catbehavior/my-cat-keeps-peeing-on-everything-but-only-sometimes-he-will-pee-in-his-litter-box-is-this-a-sereous-problem/?search=peeing
If your cat is STILL, since October 8, urinating outside of the litter box, either nothing has been done to remedy the possible behavioral issues or he does have some serious type of medical issue, like kidney problems or a UTI of some sort. I would absolutely have him checked out with your vet.
If it isn’t medical:
Have you taken care to VERY THOROUGHLY cleaned all of the spots he has urinated? If you don’t completely remove the scent, he will continue to think that is an appropriate place to eliminate. Thus, no matter what other measures you’ve taken to correct it, he’ll keep going back to those spots to urinate.
Are you ensuring that the litter box is kept clean? Sometimes a cat will shun its litter box if it isn’t kept up to its standards of cleanliness. He’s using it consistently to defecate, though? If so, location and cleanliness likely wouldn’t be the issue.
There are actually quite a lot of threads on this website about inappropriate elimination - do a quick search & you will find TONS of information on this topic.

You need to get a good pet oder cleaner. Try Pettastic. It work really well at breaking down the order. Your cat will go back to those places unless you get rid of the oder so he can’t smell it. That means you have to have a cleaner that breaks down the emzimes. You need to make sure your cat doesn’t have kidney problems or UTI. So have it checked out at the vet. When you are not around confine the cat to a small room like your bathroom. Also the room should not be carpeted. Hope this helps. Also with the Petastic don’t be afraid to use it full strength.
